RALEIGH -- A North Carolina doctor has been charged with second-degree murder after police say he caused a wreck that killed a 20-year-old ballerina.

Multiple media outlets reported Tuesday that Raymond Dwight Cook of Raleigh surrendered his medical license before appearing in a Wake County courtroom for a hearing in connection with death of Elena Bright Shapiro, a 20-year-old dancer with the Carolina Ballet.

Cook's vehicle struck the back of Shapiro's car Friday night after he was traveling 85 miles per hour in a 45 mph zone. The ballerina from Winston-Salem later died.

Cook's bond was set at $250,000 on Tuesday. He has agreed to enter a substance abuse program.

In addition to surrendering his medical license, Cook resigned his $288,577 a year post as assistant professor in the UNC Chapel Hill School of Medicine department of otolaryngology and head and neck surgery.

He was also employed at WakeMed Facial Plastic Surgery in Raleigh.

Cook graduated from medical school at UNC Chapel Hill in 1997 and completed his residency at Duke Medical Center.

He had been convicted of drunken driving in Georgia in 1989 and a year earlier had been charged with drunken driving but the charges were dropped.
