Senior ‘CommUnity Care’ center opens

Senior CommUnity Care of North Carolina (SCCNC) has opened a Program of All-Inclusive Care for the Elderly (PACE) in Durham. It provides health care and social services for frail individuals who need nursing home care but wish to remain at home.

Services are provided at the SCCNC center by health-care and social-service professionals who work together as interdisciplinary teams to provide and plan care for seniors in the PACE program. Transportation is provided to the center, medical appointments and other specialty care as needed.

Health care provided at the SCCNC center sometimes is supplemented by in-home services. 

The Durham facility at 4022 Stirrup Creek Drive, Suite 315 in Durham.
